The Behavioral Clinical Care Manager (LCSW) is responsible for direct clinical/patient care, coordination of referrals, communication among providers, maintenance of database for care management and tracking outcomes.
In this role, the selected candidate will direct the day-to-day functions of the facility in accordance with current federal, state, and local standards, guidelines and regulations that govern long term care facilities, as well as all company policies to assure that the highest degree of quality care can be provided to the residents at all times.
Act a resource, provides leadership, administration and support to HMHP Continuum Navigators, HMHP ED Care Coordinators and Social Services to ensure achievement of HMHP strategic and operational plans.
Develops specific outreach plans for assigned members who do not maintain regular contact with their medical or behavioral health provider as recommended contributing to frequent crises, recidivism, and interfering with maximum benefit from available care.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
