Coordinates all aspects of the patients' care; interacting with other disciplines as needed/required, and provides professional care to the patient as indicated by agency policies, State and Federal laws and regulations.
In this role, you will be responsible for the coordination of the patient's care and services. Promotes quality care using a collaborative process that coordinates, monitors, and evaluates services according to the needs of and interpersonal contact with patients.
Demonstrates capability to read, understand and apply standard to moderately complex documents affecting real estate projects, including but not limited to: agreements/contracts, leases, work letters, project charters, surveys and drawings.
The Nurse Care Manager provides patient behavioral and medical assessment, eliminates care gaps, works with the core Community Care team and partners such as physicians, pharmacies, home health care, hospice, skilled nursing facilities, inpatient rehabilitation facilities and universities.
