The Home Health Case Manager is a RN possessing special knowledge of care practices and regulations required for provision of comprehensive care to patients in their homes, managing the care plan for a group of assigned patients.
The RN Case Manager will help to educate patients and their families on how to follow their care plans as well as provide nursing care to patients. In this position you will act as social workers, advocates to patient welfare, and serve as a liaison between patients, their families and healthcare providers.
The Nurse Case Manager, utilizing advanced clinical judgment and critical thinking skills, is responsible for telephonically assessing, planning, implementing and coordinating all case management activities with members to evaluate appropriate physical and behavioral healthcare and social services for the member.
The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
