The clinical social worker provides psychosocial assessments, interviews, and/or treatments and discharge planning to patients and families to assist them in coping with hospitalization, illness, diagnosis, and/or life situation.

Responsible for providing an array of group therapy services to adult psychiatric and chemically dependent patients, while coordinating specific aspects of individual patient treatment in the role of primary counselor.

In this role, you will be responsible for coordinating services to minimize fragmentation of care, reduce barriers, and link children with appropriate services to ensure comprehensive, continuous access to needed medical,social,educational, and other services appropriate to the needs of the child.
In this role, you will document all findings, contracts and interventions per protocol. The selected candidate will coordinate necessary services to participating ancillary service providers and public agencies as appropriate.
