In this role, the selected candidate will provide home-based, solution focused case management support and/or crisis interventions to individuals and families both as primary and back-up family worker.
In this role, you will provide comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate. Primary assignment will be in Ambulatory Medicine with additional duties as assigned.
In this role, the candidate will be responsible for providing bio-psychosocial mental health assessments and crisis intervention services to patients of all ages including their families/support system and recommending the treatment plan to physician.
In this role, you will administer and interpret tests and measures of psychosocial functioning for the purpose of diagnosing mental, emotional, behavioral, addictive and developmental disorders and disabilities.
In this role, you will be responsible for intervening with patients and families, by applying critical thinking skills to monitor psychosocial status and issues that impact the clinical progression and transition/discharge plan for patients, including the facilitation of patient and family communication, coping and decision making Works as a part of a multidisciplinary team.
