The dialysis lifestyle can be very trying for our patients. By forming relationships with them, our nurses understand each patient's unique needs and help ease some of their burden.
As a Peritoneal Dialysis Registered Nurse (PD RN), you educate your patients on how to do their own dialysis from their homes, and you take responsibility for the complete ongoing care of each patient to ensure their treatment stays ontrack.
In this role, you will assesses home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.
In this role, you will develop an outcome oriented plan of care that reflects the patient's actual or potential health problems. Implements the plan of care to achieve the identified outcomes for the patient according to policy and procedure.
In this role, you will serve as liaison between the hospital, patient and patients family in resolving patient care problems. Coordinates service recovery efforts through complaint investigation and resolution.
