This position provides comprehensive care coordination for patients as assigned. This position assesses the patients plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patients health care needs.

In this role you will develop strategies to facilitate an injured employee's return to work and achieve maximum medical improvement. The Selected Candidate will evaluate and update treatment and return to work plans within established protocols throughout the life of the claim.
Responsible for helping to ensure injured parties receive appropriate treatment directly related to the compensable injury or assist claim handlers in managing medical treatment to an appropriate resolution.
The Case Manager is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file found on Southwest Key Programs (SWK) designated web-based program with supporting documentation maintained in physical files.
