Support providers to assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
Confer with physician in developing a plan of care based on physicians orders and initial patient assessment; provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Managers supervision.
The Nurse Navigators primary function is to build a relationship with patients and physicians, to coordinate a plan of care including appointments, transportation, education, provision and / or enablement of support services and representation within the multidisciplinary care environment.
A registered professional nurse is responsible for delivery of patient care through the nursing process of assessment, planning, implementation and evaluation. Also directs and guides patient/family teaching and activities of ancillary personnel while maintaining standards of professional nursing.
In this role, the selected candidate will be responsible for the timely and accurate completion of the MDS process from admission to discharge. Coordinate information systems operations and education for the clinical department.
