In this role, the successful candidate will be responsible for telephonic and face to face assessments, planning, implementing and coordinating all care management activities with members to evaluate the medical and disability needs of the member to facilitate the member's overall wellness.
In this role, the selected candidate will identify and resolve issues affecting the delivery of patient care services for the assigned unit(s); develop and implement policies and procedures to ensure efficient and effective delivery of health services in a unit.
