The RN Case Manager provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment. Coordinate the integration of the social service function into patient care.
In this role, the selected candidate will be performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
In this role, you will manage one-time task assignment or full field case management referral to meet with attending physician, injured worker and/or employer to clarify medical and/or vocational claim related information which could not be obtained through correspondence or telephonic attempts by claims team.
The rn-case manager works closely with the collaborative care team, assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care in order to meet the needs of the individual(s).
Provide a wide range of nursing care to patients in the home care setting. Focus primarily on patient assessment, quality care, teaching of patients and caregivers, and performance of skilled procedures.
In this role, you will be responsible for prioritize care needs, set goals and develop an initial plan of care that also addresses gaps and / or barriers to care and uses evidence-based practice as the foundation.
