In this role, you will work with agency staff, physicians, and other professional referral sources to ensure that patient services are delivered in a safe and effective manner, processes are followed, coordination of care is evident, and customer satisfaction is high.
In this role, you will provide input to care plan development and inform the Company location clinical staff (e.g. Intake Specialist, Director Clinical Management, and/or Manager Clinical Practice) of patients acceptance, and ensures that patients needs are appropriately matched.
In this role, the selected candidate will be responsible for coordinating all point of care testing according to manufacturers procedure, CLIA requirements, and Joint Commission and CAP accreditation standards.
In this role, you will be responsible for members gaining access to needed services through coordination and integration of medical and long term care services for the purpose of orientation, care plan development, assessment, and care coordination.
In this role, you will conduct screening and risk assessment interventions per program guidelines. Provide outreach and preventative care to members seeking services in the least restrictive environment.
