In this role, you will provide input to care plan development and inform the Company location clinical staff (e.g. Intake Specialist, Director Clinical Management, and/or Manager Clinical Practice) of patients acceptance, and ensures that patients needs are appropriately matched.
In this role, you will be responsible for members gaining access to needed services through coordination and integration of medical and long term care services for the purpose of orientation, care plan development, assessment, and care coordination.
In this role, you will assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care; utilize both company and community-based resources to establish a safe and effective case management plan for members.
In this role, the selected candidate will enhance and promote the agency in the marketplace by providing information and education to professionals, facilities, organizations, and consumers regarding the services provided as well as the coverage criteria and reimbursement for these services.
In this role, you will be responsible for the observation, assessment, nursing diagnosis, planning, intervention, and evaluation of care; health teaching and counseling of the ill, injured, or infirm; and the promotion of wellness, maintenance of health, and prevention of illness of others.
