In this role, you will be responsible for quality management of healthcare issues for patients in designated primary or specialty area, including patient diagnoses, treatment, education and referrals to ensure optimal patient medical management consistent with the Parkland mission of patient centered/patient valued care.
Provides comprehensive nursing care for assigned patients under the direction of a physician. Follows established nursing standards, procedures and practices, and gives patient care directions to nursing and other staff.
Serve as an advanced practice nurse providing primary level of care in collaboration with physicians and other members of the health care team to a specific patient population in the outpatient and/or inpatient setting.
The Acute Care Nurse Practitioner/Adult-Gerontology Acute Care Nurse Practitioner (ACNP/AGACNP) OR The Physician Assistant (PA) develops and maintains an active practice based on an expanded knowledge base of pathophysiology and pharmacology to provide comprehensive health assessment, diagnosis, and treatment of individuals with cardiothoracic surgery needs in collaboration with and under the direct supervision of attending physician(s).
The ideal candidate will perform all aspects of patient care, including diagnosis, treatments, and consultation with our transplant patients. As an integral part of our Transplant team will be performing the important task of educating patients about the disease process, transplant evaluation process and post-operative care following transplant.
Serves as the primary coordinator of all disciplines for well-coordinated patient care.
Monitors, records and communicates patient condition as appropriate utilizing computerized documentation systems.
