The Diabetes Self-Management Training/Education Clinical Coordinator (DSMT/E CC) is a member of the Community Health Center and within the Administration-DSRIP department. This position serves as a resource for Self-Management Lifestyle education to Company's Inpatient, Outpatient, and Community population.
