The Registered Nurse RN assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. The RN evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.
In this role, the selected candidate will prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.

Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing and company policies when called upon after hours.
You will apply the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning. Effectively communicate pertinent patient/family information to the health care team.
As directed by Clinical Supervisor, while in Case Manager role, is responsible for other Registered Nurses, Licensed Practical Nurses, and Home Health Aides who may be working on a particular home care case for which the primary RN is responsible for managing.
