Coordinate pre-admission and outpatient registration services for patients across the continuum of care, promoting quality care and effective utilization of resources; provide leadership when dealing with patient pre-admission functions, including inpatient and outpatient services.
The RN Unit Registered Nurse delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards.
As directed by Clinical Supervisor, while in Case Manager role, is responsible for other Registered Nurses, Licensed Practical Nurses, and Home Health Aides who may be working on a particular home care case for which the primary RN is responsible for managing.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.
The Dialysis Registered Nurse is key to providing specialized nursing care in the in-center hemodialysis unit; the RN works in collaboration with all clinic team members to ensure every patient receives the safest care with the highest quality optimal outcomes.
