Acts as resident/family advocate and ensure the resident is knowledgeable in and exercises his/her rights. Responsible for organizing and conduction of family counsel meetings and maintaining minutes of the meeting.
Provide psychosocial services designed to foster
coping, support optimal functioning and maximize the patients utilization of available resources; assist patients and their families in accessing
financial, pharmaceutical, and transportation assistance programs.
Assess the psychosocial status of patients and families / caregivers related to the patients terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
In this role, the selected candidate will be responsible to performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Independently implements treatment modalities.
