The Registered Nurse/POC RN in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies.
A Registered Nurse I is a licensed clinician who coordinates and provides direct patient care in the home or alternate treatment site to ensure patient safety with continuity and compliance under a physicians plan of care.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Direct or supervise less-skilled nursing or healthcare personnel or supervise a particular unit. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.

In this role, the selected candidate will prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
This individual will assess patient and document findings, administer medications, as well as assess and address patient/family educational needs regarding end-stage renal disease, dialysis therapy, diet and medications.
