The role of the Acute Care Coordinator is to serve as an adjunct to the multidisciplinary team.Coordinates use of clinical and ancillary resources within and outside of the health system to achieve treatment goals specified in the patient care plan and facilities discharge.
In this role, you will manage individualized, goal-directed nursing care through use of the nursing process and the principles of primary nursing in accordance with departmental and hospital policies and procedures.
The Registered Nurse (RN) protects, promotes, and optimizes health and abilities, prevention of illness and injury, and the alleviation of suffering through the diagnosis and treatment of human response and through advocacy in the care of individuals and families.
In this role, you will be responsible for the assessment of patient and family needs, and the development, implementation and evaluation of an appropriate (goal directed) plan of nursing care, including assessment for the need of aide service.
The Ambulatory RN professional nurse is accountable for the delivery of safe patient care utilizing the nursing process. The RN assesses, plans, delivers, coordinates, integrates, and evaluates nursing care related to patient needs.
