Utilizes FMS Ultra Care patient education programs, established social work theory and methods and quality of life measurement instruments as part of assessment and care planning to resolve barriers and meet patient treatment goals.
Responsible for performing prior authorization review of services that require notification, using applicable benefit plan documents, evidence - based medical policy and state specific recognized clinical guidelines and criteria.
Possesses and applies the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Provide psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice.
Provide therapeutic services to children on a weekly basis and families on a bi-weekly basis using differential treatment modalities; work with interdisciplinary team to develop a comprehensive treatment plan and to monitor ongoing progress of child and family.
Training and experience necessary to address the specific needs of the patients served in assigned areas of responsibility; this includes issues relating to age and stage of development and the particular special needs and cultural patterns of the patient populations being served.
