The Hospice RN Case Manager is a skilled professional member of the Interdisciplinary Team, responsible for implementing the nursing component of the plan of care and coordination of other clinical and support services.
The RN Case Manager will manage each patients care efficiently and effectively to achieve positive patient outcomes, and be responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPN, HHA) who will be directed / supervised by the RN.
The Care Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and / or health needs through communication and available resources to promote optimal, cost - effective outcomes.
