The RN UNIT REGISTERED NURSE delivers quality nursing care to patients through interpersonal contact and provides care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being in accordance with all applicable laws, regulations, and Life Care standards.
The Hospice Nurse - Registered Nurse (RN) After Hours will provide nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care.
Develop and implement a safe, realistic, individualized Plan of Care for each patient in collaboration with other services/disciplines according to best practice standards to promote positive patient outcomes and patient satisfaction using a patient-centered approach.
The Nurse Coordinator will conduct comprehensive physical assessments for every client upon enrollment to CAR Team services and will work in conjunction with Health and Wellness Coordinators to conduct psycho-educational groups and other relevant training to both clients and staff on annual basis.

Services are highly individualized and are delivered by a mobile, community based, multi-disciplinary team of highly dedicated staff who work with people to meet their unique rehabilitation needs, improve functioning and enhance their roles within the community.
