This position is responsible for working with foster and biological families in their home communities by offering comprehensive case management services. The goal of the position is to implement services for the long-term, leading to positive outcomes for the youth and families served.
Be responsible for all paperwork including, but not limited to, treatment plans and reviews, all reports, and comprehensive assessments are done according to state and requirements. Documents are to be complete and submitted on time.
Responsible for working with Emergency Department staff to create, educate and implement substance abuse processes, treatment modules and evaluations for ED patients presenting with substance abuse concerns.
The case manager will provide assessment, quality case management, and psychotherapeutic services to all adult and adolescent patients and their families serve as a member of the interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and families.
The Case Manager communicates with the parent / legal guardian and any other involved community service provider, and with the childs input, develops a service plan for the duration of the childs stay at Healy House.
Responsible for performing direct clinical and administrative support for an adolescent residential service program, including clinical case management, advocacy, education, and trauma informed services.
Communication with claimants, medical professionals, claims staff and others to obtain information necessary to make sound medical decisions. In addition, responds to various written, telephone and/or in-person inquiries regarding status of case.
The RN Case Manager promotes the optimal health and well being of patients, their families and caregivers within their homes and communities using a holistic approach that empowers patients/families/caregivers to achieve their highest levels of physical, functional, spiritual, and psychological health.
