In this role, you will provide clinical support to the sales team as it pertains to the production of new clinical programs and/or marketing activities to physicians, nursing agencies and referral sources.
In this role, you will performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
The Case Manager is responsible for assessing patient/family psychosocial needs, planning, implementing, procuring services for and evaluating care for patients ranging in age from 18 years to geriatric experiencing medically complex conditions, medical-surgical conditions with rehabilitation and/or wound care needs.
In this role, the successful candidate will identify gaps in services for nursing care and proposes solutions to problems identified; act as a liaison and advocate for patient services or project participants.
