The Care Coordinator functions as a core member of a collaborative care team that involves the patients primary care provider, a consulting psychiatrist and other mental health providers that may be available in the primary care clinic.
In this role, you will be responsible for supervising, implementing, coordinating, and managing patient care through interpersonal contact with patients, families, nursing staff, and others on his/her respective unit in accordance with all applicable laws, regulations, and Life Care standards.
In this role, you will identify special needs members through the completion of health screens and other resources; assist with development of plan specific literature and education materials in conjunction with medical director and corporate oversight.
The Patient Care Coordinator is a member of a dynamic team focused on coordinating all aspects of patient care. The role provides continuity of care by ensuring a smooth transition between care setting and by developing a trusted relationship with patients to identify and assist with each patients individual needs that may arise related to the care they receive.
