The selected candidate will perform the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the clients care.

In this role, you will be responsible for Enterprise wide care management functions across all contracted SNFs to ensure regulatory standards are met and appropriate utilization of SNF care and coordination across the continuum occurs.
In this role, you will implement and updates Nursing Service policies and procedures in cooperation with the Medical Director to meet Sea Mar standards, philosophy, and governmental regulations. The person in this position will select, supervise and evaluate nursing personnel.
The Nurse Manager is responsible for managing and directing the activities of patient care in accordance with the established standards of care, mental health practice and facility policy, goals, and objectives on their assigned unit.
The primary responsibility of the RN Case Manager is to identify, screen, track, monitor and coordinate the care of patients with multiple co-morbidities and/or psychosocial needs and develop a case management plan of care for high-risk patients.
