In this role, you will be responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patients resources and right to self-determination.
The Social Worker will provide crisis intervention and support. The Social Worker will assist the Case Manager in facilitating patient movement across the continuum of care and will identify and track barriers to patient throughput.

The Licensed Social Worker will assist in the planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards as well as our established policies and procedures to ensure that the medically related emotional and social needs of patients and/or residents are met and maintained on an individual basis.
