Provide clinical recommendations pertaining to, but not limited to, gaps in care, high risk medications, compliance and adherence, drug interactions, therapeutic substitution, and generic substitution.
Clinical pharmacist specialists may serve as preceptors for pharmacy students and residents. Clinical pharmacist specialists have advanced training or experience in relevant areas of specialized practice.
The pharmacist assures the appropriate selection, dosing, administering, and dispensing of medication orders. Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
Work collaboratively with the pharmacy team and other health care providers to achieve desired patient outcomes through safe, timely, and cost-effective drug therapy. Performs the duties of staff pharmacist as required.
