The Nurse Practitioner (NP) 2 works as part of the Clinical Operations Team, providing clinical assessment and care to assigned patients in their homes and in skilled nursing facilities; identifying quality improvement activities and planning individualized health care consistent with Matrix Medical Network policy protocols.
Provides direct patient care services. Elicits detailed patient histories, assesses patient medical needs, initiates appropriate diagnostic testing and/or treatment and completes appropriate medical record documentation.
This Nurse Practitioner( NP ) position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost - effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients.
Knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required. Knowledge of the administrative requirements for proper documentation of patients condition including disease progress, acknowledgement of teaching, and follow up care.
Perform comprehensive medical assessments and document findings in a concise / comprehensive manner. Provides basic primary medical care and treatment to patients in their homes under direction of a collaborating physician.
