In this position, you will work with Care Coordination MVP Team members to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize the health of the Member. Coordinates, monitors and ensures that appropriate and timely primary, acute and long-term care services are provided to members across the continuum of care.
In this role, you will provide a complete range of womens services, including its acclaimed maternity unit, maternal-fetal medicine, a high-risk maternal and neonatal transport team, the regions largest and most sophisticated neonatal intensive care.
Works in collaboration with the Department Head to identify educational needs of staff in the specialty area, to develop programs to meet these needs, and evaluate outcomes. Orients new staff, trains and serves as a mentor for preceptors, assures competency validation of new staff and facilitates annual competency validation updates
In this role, you will collaborates with multidisciplinary team to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures.
