The purpose of the Clinical Nurse Educator is to provide point-of-service support with key healthcare providers, clinics, and infusion centers within a specified geography in order to enhance disease knowledge, support the patient therapy selection process, coordinate and provide training, assist patient pull through, enhance compliance and improve adherence.
In this role, you will be responsible for cardiac patients, titrating multiple vasoactive medications, invasive hemodynamic monitoring, ventilators and rapid weaning, intra-aortic balloon pumps, CRRT, and ventricular assist devices.
In this role, you will assess and coordinate patient's discharge planning needs with members of the healthcare team. The selected candidate will serve as a primary coordinator of all disciplines for well - coordinated patient care.
Provide educational support to identified patients on the infusion technique for approved medication and provide ongoing support to support adherence to therapy and ensure patients are connected to the appropriate support services offered by the client.
The Nurse Practitioner provides Psychiatric and Mental Health care through case and medication management and serves as a liaison and advocate for patients by assisting in coordinating continuing care for those with acute and chronic psychiatric problems.
