In this role, you will evaluate and perform ongoing assessment and revise initial written plan of care with Interdisciplinary collaboration weekly or as the needs and conditions of the patient/family change.
Responsible for the development of the M/Cs interdisciplinary individualized plan of care including short term and long term goals in collaboration with the M/C. Monitors and evaluates the M/Cs progress towards meeting goals and initiates appropriate interventions as necessary.
Oversee the frequency of visits for the episode, in addition to performing visits and completing coordination of client care. Consult as needed with the physician and the office giving details about patient care.
As a Peritoneal Dialysis Registered Nurse (PD RN), you educate your patients on how to do their own dialysis from their homes, and you take responsibility for the complete ongoing care of each patient to ensure their treatment stays on track.
The PHN performs the Personal Health Management process, that is assesses the participant, works with the participant, family and physician to identify problems, establish goals and develop plans of care, coordinates services, educates participants, empowers participants to independently self-manage and to make knowledgeable health care decisions.
