Promotes self-awareness and knowledge of current medical and case management standards in the community, recent innovations in patient care, and availability of alternative community, local, state, and federal resources.
Oversee the frequency of visits for the episode, in addition to performing visits and completing coordination of client care. Consult as needed with the physician and the office giving details about patient care.
Responsible for the development of the M/Cs interdisciplinary individualized plan of care including short term and long term goals in collaboration with the M/C. Monitors and evaluates the M/Cs progress towards meeting goals and initiates appropriate interventions as necessary.
In this role, you will evaluate and perform ongoing assessment and revise initial written plan of care with Interdisciplinary collaboration weekly or as the needs and conditions of the patient/family change.
The Case Manager II will provide case management services to patients and their families or caregivers. Assist with assessment, participate in planning and implementation of intervention and ensure follow-up and coordination of services.
