In this role, you will develop and implement educational materials for departments and assures processes established for patients and families to receive appropriate educational counseling to assist them in physical and psychosocial progress towards outcomes.
In this role, you will perform patient assessments including: biophysical, psychosocial, spiritual, cultural, and age appropriate needs of the patients, including the assessment of nutrition, pain management, and signs/symptoms of abuse or neglect in order to develop, evaluate and modify patient's plan of care.

In this role, you will perform a complete assessment of assigned patient's ongoing physical and emotional status, level of competency, home environment, safety factors and family support available to assist with care and needs of patient.
