In this role, the selected candidate will perform the networking aspect and linkages to other needed services, internally, and externally;
supervise direct day to day activities, individual treatment plans and updates as needed according to policy.
The Case Manager performs general and high risk, complex Case Management activities across the continuum of care to drive and support proactive quality, cost efficient outcomes and minimize fragmentation of health care delivery.
In this role, the selected candidate will provide day to day oversight and coordination
of case load to ensure all case elements and tasks are completed timely and ensure cases move through the process as required.
The Complex Case Manager functions across the continuum of telephonic Case Management, with the overall goal of activating the consumer to enhance quality of care, to facilitate change in consumer behavior and to promote optimal outcomes for all parties involved.
In this role, you will be completing paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, treatment/recovery plans, progress notes and any other funder requirements.
In this role, you will ability to manage client care with specific knowledge and experience in bedside care, symptom management, crisis intervention and family intervention are feature assets for this high-profile nursing position.
In this role, the selected candidate will work telephonically with patients identified as high risk, for both behavioral and physical health issues, and their providers to identify needs, set goals and implement action steps towards achieving goals.
