The Bariatric Program Dietitian has a dual reporting relationship, they will report to the Director of Nutrition Services or designee for clinical support and to the Bariatric Program Manager for non-clinical support.
Participate with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patients needs, in accordance with established Federal and, where applicable, state licensure guidelines.
Provide comprehensive nutrition assessments and care planning for patients/residents and instructs patients/residents and/or hospital personnel in principles of nutrition and modifications of normal diet.
Provides medical nutritional therapy to patients by assessing nutritional status, developing a plan of care to maintain or improve nutritional status, periodically evaluating the patient's response to the plan of care, and modifying it as required for optimal outcomes.
Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
