Instructs patient/family/care provider as appropriate on recommended nutrition/ dietary modification using an individualized approach.
Assists patient/family to obtain and use community resources/services to meet their nutritional needs.
The Bariatric Program Dietitian has a dual reporting relationship, they will report to the Director of Nutrition Services or designee for clinical support and to the Bariatric Program Manager for non-clinical support.
Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
Assesses patients knowledge of diet and kidney disease and provides education appropriate to patients learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
