Assists physicians in providing medical care to assigned patients. Planning and implementing physical examinations, interventions, and evaluating patient response according to instructions of the attending physician.
Care for patients from beginning to end-of-life through its 90+ clinics, 12 hospitals, 15 pharmacies, specialty care centers and specialty medical services that provide home care, senior transitions, hospice care, home oxygen and medical equipment, and emergency medical transportation services.
This position will manage the care of patients hospitalized with complex acute health problems in collaboration and consultation with other physicians, APPs, staff RN's and other health care professionals.
Position includes outpatient primary care of a diverse group of refugees and immigrants. Inpatient rounding is available, though, not required. There is also an opportunity for those interested in academic medicine to precept University of Minnesota Global Health Pathway residents who have their continuity clinic at CIH, though, not required.
Provide strategic and work direction of provider Clinical Documentation Improvement and Case Management teams; ensure appropriate medical oversight of the Utilization/Case Management and provider documentation and billing activities.
Provide clinical care for patients including assessment, diagnosis/treatment of illness, ordering and interpreting medical diagnostics, chronic illness management, wellness care, prescribing of medications, treatments, therapy, medical equipment, etc.
The NP/PA will be expected to assess, diagnose, treat, and manage male and female patients with acute and chronic urologic conditions. This is primarily an outpatient position which includes new consultations, follow-up visits, pre op, and post op appointments.
