This position is responsible for the delivery of social work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed.
Social Workers are responsible for providing high quality support and services for the children and others we serve. They utilize effective communication and interaction skills with students, children, families and others to promote success and help those we serve achieve the highest possible quality of life.
Performing activities required for the effective and efficient operation / maintenance of the Social Work professional group; performing other duties consistent with the job classification, as required.
In this role, you will be responsible for collaboration with other agencies, services, community providers as needed towards achieving discharge; notification and involvement of administrative personnel as needed to achieve a safe discharge.
In this role, the selected candidate will be responsible to provide direct clinical and consultation services in accordance with the policies and procedures of the Facility/Unit, and in accordance with the ethics and standards of the National Association of Social Work.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
