Performing activities required for the effective and efficient operation / maintenance of the Social Work professional group; performing other duties consistent with the job classification, as required.
Ensures more efficient coordination of care, streamlined patient access to specialized care when needed, appropriate management for patients, prevention of emergency department (ED) visits, and decreased inpatient unit or institutional care admissions.
This position is responsible for the delivery of social work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed.
Responsible to provide direct clinical and consultation services in accordance with the policies and procedures of the Facility / Unit, and in accordance with the ethics and standards of the National Association of Social Work.
Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
The Social Worker, in collaboration with the interdisciplinary care team, provides social work clinical services, including discharge planning, to individuals and families of all ages who come to the Emergency Department for care.
