The Manager demonstrates effective leadership of one or more patient care units or areas. He/she has twenty-four hour responsibility for assigned areas. Demonstrates operational excellence in effective management of fiscal, material, information systems, and human resources.
The RN Case Manager consults as needed with the physician and the office giving details about patient care; performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
In this role, the selected candidate will be responsible for assessing, planning, implementing and evaluating all phases of nursing care provided on each program; determines staffing requirements and material resources necessary to provide care to the patients for each program.
In this role, you will determine the appropriate place of contact/intervention with customer including client's home, community agency, provider office, etc. In order to achieve customer satisfaction and a positive outcome.
The Nurse Manager is responsible for coordinating administrative and clinical activities, both inter and intradepartmentally, so that optimum nursing care is provided, staffing is maintained, and costs contained.
In this role, the selected candidate will organize functions of the nursing home to carry out programs and meet resident needs within the framework of the mission and philosophy of company and Federal, State, and Local regulatory guidelines.
