Position assists the unit manager in supervision, coordination, education, and management of nursing personnel to ensure consistent standards of practice and competencies in the delivery of patient care services on an assigned shift.
The RN Coach will function in a telephonic virtual care center and is part of the Care Connections Team which collaborates with physicians, mid-level providers, staff, and other health care professionals to provide coordination of care across the health care continuum.
The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status.
Responsible for the overall structure of the Ortho/Spine surgery program coordination including achieving and maintaining accreditation as a Center of Excellence from the American Academy of Orthopedic Surgeons and other credentialing bodies as deemed appropriate.
Manage care/services provided and the development/implementation of patient plans of care for a group of assigned patients within scope of practice, to include performing on-site clinical assessments as appropriate, consulting with third party representatives regarding patient financial responsibility, providing case management and follow up, to ensure care plan stated outcomes are achieved.
This individual will assess patient and document findings, administer medications, as well as assess and address patient/family educational needs regarding end-stage renal disease, dialysis therapy, diet and medications.
Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Clinical Manager and initiate as appropriate.
