In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Our Registered Nurses are responsible for providing IV nursing care in the home or ambulatory setting, performing in-hospital discharge assessments and occasional IV hook-ups, teaching patients about their home infusion therapy, and providing ongoing monitoring of their treatment.
Assess and evaluate patient health care data collected through interviewing, observation, record review, and peer and team input in order to use sound clinical judgment in the treatment planning process.

Responsible for providing quality nursing care to patients of the Cardiac Cath area while utilizing the nursing process, and acts as a liaison between the Cardiac Cath area and the nursing units to provide continuity of care and optimal communication.
Coordinate total nursing care for patients, participates in patient and family teaching and provides leadership by working cooperatively with other nursing staff and multidisciplinary team members in maintaining standards of professional nursing practice.
The RN documents all aspects of patient care and the patient's response to care according to established policies and procedures and communicates in a professional manner, providing information to physicians, other staff, and patients/families as appropriate.
Responsible for delivering professional nursing services, either directly or indirectly, for a defined group of patients according to emergency department and hospital policy. 12 hour shifts Rotating Weekends.
