In this role, you will oversee the frequency of visits for the episode, in addition to performing visits and completing coordination of client care; ensure lab values have been reported to the physician.
In this role, you will be responsible for initial case evaluation by assessing injury/disability severity, treatment plans, functional abilities and physical job requirements. Work wit h vocational rehabilitation where appropriate for return-to-work planning.
In this role, you will be responsible to
develop and modify treatment plans; review and evaluate patient progress; handle patients in crisis; coordinate counseling efforts; establish treatment goals; participate in interdisciplinary treatment team; and provide patient and family education.
The Case Manager proactively identifies and assesses high-risk Maternity members with complex conditions, and develops and implements an individualized, coordinated care plan, in collaboration with the member and Primary Care Physician and/or specialist, to ensure a cost effective, quality outcome, focused in the ambulatory setting.
