Performing visits and completing coordination of client care, the Registered Nurse (RN) Case Manager is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode.
Perform duties related to the day-to-day operations of the High Risk Case Management functions to include working with members identified as high risk to identify needs and goals to achieve empowerment and improved quality of life.
The case manager will provide assessment, quality case management, and psychotherapeutic services to all adult and adolescent patients and their families serve as a member of the interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and families.
Responsible for working with Emergency Department staff to create, educate and implement substance abuse processes, treatment modules and evaluations for ED patients presenting with substance abuse concerns.
Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
The case manager is accountable for coordinating and facilitating the management of care for an assigned group of patients to ensure provision of a well coordinated individualized care experience through the delivery of outcome oriented patient care within a fiscally responsible framework.
The RN Case Manager utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status for our Americas International membership.
