In this position, you will collaborate with clinical and non-clinical staff and other members of the patients health care team to develop an individual plan of care to facilitate an immediate action plan to meet the patients current needs.
In this role, you will provide and implement individual, family and group psychotherapy as required. Participates in clinical after-hours on-call rotation, providing prompt crisis support by phone or in-person as required.
In this role, the selected candidate will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the inter/multi-disciplinary Team (IDT).
In this role, you will develops, implements and monitors a comprehensive plan of care through an interdisciplinary team process in conjunction with the patient and family in internal and external settings.
In this role, the successful candidate will be accountable for utilizing the nursing process to provide safe, age-appropriate, direct and indirect patient care, while practicing within the scope of practice of the Oregon State Board of Nursing.
