In this role, you will assist in the finding of satisfactory solutions or adjustments to medical, social, emotional and financial problems related to illness or disability which may interfere with access to and use of medical care, taking into account patients physical or age-related needs.
In this role, you will assess patient for medical, mental health and chemical dependency risk factors, health status, self-management skills and confidence level, knowledge of health care needs and knowledge of prescribed medications.
In this role, the selected candidate will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the inter/multi-disciplinary Team (IDT).
In this role, you will provide administration of medical management programs that include: prior authorization, concurrent hospital and skilled nursing review, appeals and grievance, delegation, medical policy development and medical claims audits.
In this role, you will facilitate services provided to enable patients and significant others to more effectively deal with the impact of medical and/or psychiatric illness, or disability, on individual and family functioning.
In this position, you will collaborate with clinical and non-clinical staff and other members of the patients health care team to develop an individual plan of care to facilitate an immediate action plan to meet the patients current needs.
