In this role, you will educate your patients on how to do their own dialysis from their homes, and you take responsibility for the complete ongoing care of each patient to ensure their treatment stays on track.
In this role, you will establish a collaborative relationship with the client (plan participant), family, physician(s), and other providers to determine medical history and current status to assess the appropriate level of care and options for alternative care.
In this role, the selected candidate will communicate observations and assessments through reports, progress records and Hospice conferences utilizing current process and technology available to primary care physicians and Hospice Interdisciplinary Team members. Consult with and educate the patient, family and caregiver regarding the disease process, self-care techniques, prevention strategies, end-of-life care and process to deal with issues of ethical concern.
The Case Manager possesses strong clinical skills and a knowledge base in a specialized area, and utilizes leadership skills in coordinating and/or providing home care services for a group of patients within that specialty area.
