The RN ADMISSIONS NURSE supervises, implements, coordinates, and manages patient care through interpersonal contact with patients, families, nursing staff, and others in accordance with all applicable laws, regulations, and Life Care standards.
Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient - specific needs, assessment data and the medical plan of care.
In this role, you will provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members.
Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.
Seeking a dynamic Registered Nurse (RN) who is compassionate about the health and well-being of our patients to assess patient/family health problems and resources, taking a leadership role in the development, implementation and outcomes evaluation of a plan for nursing care and to provide professional nursing care and interventions to acutely ill adult patient populations.
The RN Navigator works collaboratively with physician specialists (both interventional and diagnostic), genetic counselors, referring physicians, other treating physicians, surgeons and other treatment navigators to ensure that the patient gets seamless care for optimum outcomes.
In this role, you will provide oversight of the activities on a nursing unit during a specific shift, including serving as a resource to staff, directing patient flow, assuring appropriate utilization of resources and providing oversight for the provisions of quality care in a manner satisfying to patients.
