This position provides comprehensive care coordination for patients as assigned. This position assesses the patients plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patients health care needs.
A Social Worker (Case Manager) is responsible for providing case coordination and a complete range of social work services, including assessment, treatment planning and clinical / therapeutic interventions in an interdisciplinary environment consistent with the employees qualifications, professional practices and ethical standards.
As an RN Telephonic Case Manager for the Kidney Resource Services program, you will be responsible for clinical operations and medical management activities across the continuum of care from assessing and planning to implementing, coordinating, monitoring and evaluating. You will also be responsible for providing health education, coaching and treatment decision support for members.
In this role, you will works closely with DCS, collaborative partners and other community resources to streamline services for assigned clients in order to ensure all reasonable efforts are made to remove barriers, alleviate substance abuse and improve the chance for family reunification.
In this role, you will complete assessment and service plan and implements services to members in accordance with their individualized service plan (ISP). Communicates and documents member's progress toward their recovery.
The case manager is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file found on southwest key programs designated web - based program with supporting documentation maintained in physical file
In this role, the selected candidate must have an experience working with individuals diagnosed with chronic conditions or pregnancy; strong working knowledge of Microsoft Word, Internet Explorer, and proficient typing skills.
