Registered Nurse is responsible for professional nursing care to patients in their home by assessing, developing, implementing and evaluating care needs in individual and continuing basis of assigned patients in keeping with agency policies and procedures and external regulations.
In this role, you will be responsible to provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer on-going care and case management for each patient, provide necessary follow-up as directed by the clinical manager.
Responsible for coordinating and providing high quality multidisciplinary health services for patients in the Region/Program consistent with Agency philosophy, policy, goals and objectives, and Standards of Nursing Practice.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
