The Program Coordinator provides clinical oversight of the daily activities and behavior modification techniques in the Stepping Stones program. This position develops the individualized treatment plans, interprets data, and directly supervises the Lead Program Specialist and Program Specialists.
In this role, you will collaborate with Project SUCCEED DOHMH staff to implement the project components including: provider and patient navigation training; patient education, practice transformation, enhanced HCV screening, Treatment Access Model, and treatment readiness and adherence support .
In this position, you'll couple your positive attitude, superb organizational skills and deep understanding of ADP to ensure smooth functioning of activities and processes in support of one or more executives.
In this role, you will assist in the planning, implementation, and evaluation of all quality program components, the planning and supervision of all residential programming, activities, and schedules and in monitoring the daily operation of assigned programs to include client fiscal matters, physical plant and equipment, and FTE utilization.
In this role, youll assume front line responsibility for quality and continuity of care. And youll educate patients, families and other health care providers, while ensuring a seamless experience for patients.
