In this role, you will evaluate and perform ongoing assessment and revise initial written plan of care with Interdisciplinary collaboration weekly or as the needs and conditions of the patient/family change.
In this role, you will provide case management to members who are identified (specifically members being discharged from hospital admissions) in accordance with companies Medical Management policies and procedures.
Responsible for evaluating patients and developing the CM plan of care, utilization management, coordination of all post-acute services, and post-discharge coordination of care with primary care physicians.
This position will complete psychosocial assessments, assist with treatment planning, monitoring patient progress, facilitating patient and family meetings, and coordinating and implementing discharge services.
