This position is responsible for overall case management through implementation and facilitation of rehabilitation services for assigned individuals and documentation of all related activities. This position is also responsible for providing community support services off site to consumers and documentation of all related activities.
In this role, the selected candidate will be responsible for providing telephonic assessment, linking, planning, monitoring advocacy and support, and case management activities to clients accessing services.
In this role, the selected candidate will work telephonically with patients identified as high risk, for both behavioral and physical health issues, and their providers to identify needs, set goals and implement action steps towards achieving goals.
The Complex Case Manager functions across the continuum of telephonic Case Management, with the overall goal of activating the consumer to enhance quality of care, to facilitate change in consumer behavior and to promote optimal outcomes for all parties involved.
