Provide and direct speech therapy services to patients, including assessment, treatment, program planning and implementation. Develop treatment plans by identifying a problem list, long and short-term goals, and methods to achieve recovery.

The Speech Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments.
Utilize professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
Provide counsel and instruction to patients, families and healthcare staff.Attend, participate in and/or conduct internal staff development programs, obtain continuing education as required by Company policy, regulation.
